For expats who are settled in Spain long-term, mutua plans from Sanitas or Adeslas offer great value with broad Barcelona networks. The biggest are Sanitas (owned by Bupa), Adeslas, Asisa, and DKV. They operate their own hospital and clinic networks, meaning your policy typically requires you to use their network providers. Within that network, coverage is comprehensive and premiums are competitive. They are the most popular choice for long-term residents in Spain.
If you're an autónomo without employer sick pay or savings buffer, income protection is strongly recommended. Spain's baja por enfermedad system for self-employed workers only kicks in after day 4, replaces only a portion of your base contribution, and requires navigating public healthcare bureaucracy. A private income protection policy pays a defined monthly benefit from day one of inability to work, giving you financial stability while you recover.
